Sept 28-Oct 30 - could extend

Hours: M & F -6:30a-3p, T & Th - 7a-4p, W - 9a-6p

Scope:

• Onsite dispensing  Yes
• Occupational Health services  Yes
• Workers' Compensation services  Yes
• Women's Health requirements for temporary/locum providers  Yes

Staffing Information 

• Health center staffing model  1.0 FTE App, 2.0 FTE MA, 0.6 FTE PT

Additional Requirements 

• Spanish speaking highly preferred

$90 Per hour

Paid Malpractice

Paid shadow training & module training

Illinois state licensure

2+ years primary care experience as an NP or PA

Occ med experience

Board Certified

DEA
