NP or PA Nephrology

Nephrology Nurse Practitioner or Physician Assistant - Glendale, AZ

We are seeking an experienced Nurse Practitioner or Physician Assistant to join a growing nephrology team. The provider will work closely with the physicians and clinical team to care for patients with acute and chronic kidney conditions in an outpatient setting.

Responsibilities:

• Evaluate and manage patients with chronic kidney disease, hypertension, electrolyte disorders and other renal conditions
• Provide follow-up care for dialysis patients
• Perform patient assessments, develop treatment plans and order appropriate diagnostic testing
• Review laboratory results and adjust medications as needed
• Educate patients and families about kidney disease, medications and treatment options
• Collaborate with physicians, dialysis centers and other members of the healthcare team
• Maintain accurate and timely medical documentation

Qualifications:

• AZ Licensed Nurse Practitioner or Physician Assistant
  
• National board certification
• Nephrology or dialysis experience required
• Strong clinical, communication and patient-care skills
