Children's Wisconsin has an opportunity for a Nurse Clinician-Diabetes. This is an exciting opportunity to engage families in an intimate setting over a span of 1-4 hours. You will be able to build relationships and help families lead their treatment and empower them to manage their care.

Hours: Work Schedule: 24 hours per week (.6) Monday, Wednesday, Friday 8am-4:30pm. We would like the initial training to be done at a .8 if possible.

Location: This position is located on Milwaukee Campus

What you will do:

The Diabetes Nurse Clinician supports optimal wellness for children and families by promoting health maintenance, guiding the management of acute illness, and helping patients navigate the long-term effects of chronic disease and disability across the care continuum. This support is provided through both in-person and electronic interactions.

Using strong critical-thinking skills and the nursing process, the Diabetes Nurse Clinician cares for patients and caregivers with complex health needs, drawing on formal education, clinical experience, and evidence-based guidelines. In partnership with families, the clinician serves as an advocate, educator, and source of support-honoring each child’s culture, values, individual needs, health goals, and treatment preferences.

A key part of the role is ensuring continuity of care. The Diabetes Nurse Clinician collaborates with inter-professional teams and coordinates services to address the needs of the whole child. This includes assessing needs, facilitating access to appropriate healthcare services, and connecting families with community resources across all states of care.

Responsibilities of Diabetes Nurse Clinician:

•          See patients in clinic as part of care team model (assist with technology, chart prepping, education visits)

•          Provide both individualized and group DSMES family education

•          Facilitate creating self-management age specific goals

•          Manage e-mails (my chart), faxes and voicemails

•          Complete prior authorizations

•          Facilitate dose adjustments

•          Provide patients/families with assistance in obtaining prescriptions, supplies, and medication

•          Create and update electronic school plans

•          Provide education to hospital staff through consultation or in-service education 

•          Be a resource for families on questions related to glucometers, CGMs, insulin pumps


What you will need:

Associate's Degree in Nursing required
- Bachelor's Degree in Nursing preferred

Experience:

- 2+ years Licensed nursing experience required
- 2+ years Pediatric nursing experience preferred
- Experience managing patients with chronic and complex family and health needs preferred

Knowledge, Skills and Abilities

- Demonstrate proficient pediatric assessment skills, utilizing a developmental approach to meet the individual needs of the neonatal, infant, toddler, pre-school, school age and adolescent and or young adult patient.
- Ability to work with patients who have intellectual and physical disabilities in a developmentally appropriate manner.
- Proficiency with the use of computers and an electronic medical record.
- Independent judgment and analytical ability to problem solve, make decisions and formulate reports.
- Requires effective and collaborative interpersonal and communication skills for discussing sensitive issues with patients and caregivers.
- The ability to represent the organization and serve consumers in a professional manner and promote a positive image of the organization and its services.
- Expertise in active listening and therapeutic communication.
- Requires critical reasoning and astute clinical judgment to expedite appropriate care and treatment.

Licenses and Certifications:

- BLS-Basic Life Support Healthcare Provider - American Heart Association/American Red Cross/National Safety Council required
- + one of the following: - N/A
- License RN-Registered Nurse (30) - State of Wisconsin
