Job Description - Nurse Coordinator

Acts as a liaison between providers, departments, other healthcare professionals and schedulers to coordinate testing, scheduling, referrals etc. Develops and implements strategic interventions across the system to improve patient access and navigation.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).

2. State criminal background check and Federal (if applicable), as required for regulated areas

3. Obtain certification in Basic Life Support within 30 days of hire date.

EXPERIENCE:

1. Two years’ experience as an RN OR five years’ experience as an LPN required.

PREFERRED QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Bachelor's Degree in nursing(BSN).

EXPERIENCE:

1. Experience in outpatient clinical setting.

2. Proficient with Epic Cadence/Scheduling.

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position.  They are not intended to be constructed as an all-inclusive list of all responsibilities and duties.  Other duties may be assigned.

1. Attends conferences or educational opportunities to ensure up to date knowledge. 

2. Act as point of escalation for  providers, providing triaging and medical advice where applicable.

3. Coordinate scheduling, testing, and referrals across multiple departments to ensure patients move efficiently through their care pathway.

4. Monitors and manages patient’s appointments, testing and labs necessary for the patient population. 

5. Partners with specialty leaders and enterprise resources to identify areas of opportunity for improvement of access and navigation

6. Develops and implements strategic interventions across the system to improve patient access and navigation

7. Review patient charts to identify outstanding testing, labs, or clinical prerequisites and ensure completion before appointments.

8. Facilitate accurate and complete referrals, ensuring required documentation and clinical information are in place before scheduling.

9. Document coordination activities and communication in the EHR, using Epic tools and work queues to track needs and progress.

10. Participate in relevant meetings or workgroups focused on patient access, navigation, and workflow optimization.

1. Heavy/Hard work: Work requires strength and/or stamina, lifting, moving, stooping, reaching, positioning of patients, standing, walking, carrying of patients, materials and equipment weighing  35+lbs.

2. Travel may be necessary in departments with multiple locations.

SKILLS & ABILILTIES:

1. Strong working knowledge of scheduling workflows and referral processes.

2. Ability to troubleshoot complex access and coordination issues.

3. Excellent communication and problem-solving skills.

4. Proficiency in Epic or similar EHR platforms.

5. High attention to detail and accuracy.

6. Ability to work collaboratively with diverse clinical and administrative teams.

7. Strong organizational and multitasking abilities.

8. Ability to identify system-level barriers and propose actionable solutions.
