Nurse Manager
Hours: Full-Time, Exempt
POSITION SUMMARY:
A registered professional nurse who will maintain the highest quality nursing care, supervision and evaluation of non-professional and professional nursing personnel and maintain a harmonious and effective rapport with and among their coworkers. Responsible for the coordination and oversight of the short-term rehabilitation neighborhoods within Woodside Lutheran Home. Works closely with the Admissions Coordinator, Charge Nurse, and Rehab Social Worker to assure smooth transitions and a positive experience for all short-term rehabilitation residents. Deals with integrity, fairness and responsibility in all actions with employees, residents, families and the community.
ESSENTIAL FUNCTIONS:
- Assist in developing clinical and administrative policies and procedures that explain the goals and objectives of the nursing department in conjunction with the Director of Nursing
- Educate residents, families and fellow staff members in measures that will promote, maintain and restore health and comfort
- Acts as point person to follow up on all family or patient concerns as well as acts as the main resource for all staff concerns as needed.
- Recognize changes in the individual residents based on nursing knowledge, in the functions of daily living, social interaction, spirituality or psychological behaviors
- Be a contributing member in the staff education process by participating and organizing in-services, sharing individual knowledge of practices of gerontology, geriatrics and specialized rehabilitation nursing procedures to provide support to fellow employees in their pursuit of professional development. Follow up on education and training staff as a result of disciplinary action
- Participate in recruitment, orientation, and development of staff including the planning, monitoring, directing and education of staff on policies, procedures, and products
- Implement performance planning activities, such as performance evaluations, expectations, identifying and setting goals. Perform employee disciplines and terminations, as necessary
- Participate in the facility QA improvement team, as requested
- Maintain compliance with all nursing regulations per federal, state and local rules and regulations
- Scheduled for on-call duty for the Nursing Home
- Maintains inventory and stocking of pharmacies automatic dispensing unit
- Supervision of licensed staff and C.N.A.s in the rehab area of the nursing department. Responsible for planning, monitoring, directing and guiding the staff and their work
- Responsible for all applicable follow up and action items as the result of family care conferences. Attends family care conferences as appropriate.
- Responsible for all applicable follow up required as a result of the weekly Medicare meetings
- Identifies the need for and oversees continuous quality improvement efforts across the rehab neighborhoods including but not limited to audits (including but not limited to admission auditing), performance improvement plans and follow up upon completion of a quality initiative.
- Responsible for monitoring and adjusting staff PPD on a daily basis and following up on patterns of overage to determine root cause.
- Reviews and compiles daily report in preparation for daily stand-up meeting and recognizes changes patient in condition. Recommends appropriate follow-up to prevent re-hospitalizations.
- Responsible for bowel and bladder program oversight, including ensuring the completion of a three-day assessment and continuous collaboration on to ensure appropriate care planning.
- Assemble information and data to determine current health status and functioning ability of a resident in order to formulate, implement, evaluate and modify a plan of care
- Develop and maintain resident care plans, C.N.A. Care Cards, audit follow through of care, medication utilization and accurate charting per set guidelines
- Active participation in Fall and Wound and Hydration Committees and other committees as required by the state and Federal codes or as assigned by the Director of Nursing
- Ensures continuous psychotropic drug monitoring including but not limited to GDRs and appropriate diagnosis
- Ensures compliance of monthly reauthorizations
- Attends hospitalization meetings and reviews/compiles data in preparation for the meetings to prevent reauthorizations.
- Responsible for utilizing a systematic risk management system to analyze and determine root cause for falls and injuries of unknown origin.

- Is knowledgeable of resident rights and ensures an atmosphere which allows for the privacy, dignity, and well-being of all residents/tenants in a safe, secure environment
- Observes care in ensuring the confidentiality of all data, including resident, employee, and operations data as required by HIPAA/HITECH regulations
- Abides by all applicable statutes and regulations through compliance with the Woodside Corporate Compliance and Ethics Program
- Completes requirements for in-service training
