Job Description - Nurse Navigator

Healthcare professional who helps patients navigate the healthcare system from diagnosis, through
treatment and sometimes end-of-life care.  This person will act as a bridge between patients and clinical
care staff providing personalized care to patients, families and caregivers.  They help with connecting
patients to medical and social resources. Follows up with patients regarding regularly scheduled
appointments and referral appointments. Provides follow up to health care providers, agencies and other
resources about patient progress towards meeting needs and/or goals.

ESSENTIAL FUNCTIONS:
 Supporting patients throughout the healthcare process
 Educating patients about their diagnoses, treatments and the side effects of these
 Helping patients to overcome any barriers to healthcare that they may face
 Providing resources for patients seeking reliable information
 Helping patients to overcome barriers such as financing or transport
 Managing timely appointment scheduling for tests and procedures
 Answering questions or liaising with doctors to address questions and concerns
 Providing emotional support and empowerment to patients and their loved ones
MINIMUM KNOWLEDGE AND SKILLS REQUIRED:
 Basic computer skills
 Demonstrates ability to communicate effectively
 Self-motivated
 Detail oriented
PREFERRED KNOWLEDGE AND SKILLS REQUIRED:
 Sound knowledge of the healthcare system and where to find reliable information
 Cultural diversity

MINIMUM EDUCATION AND EXPERIENCE REQUIRED:
 Graduate of nursing program
PREFERRED EDUCATION AND EXPERIENCE:
 Extensive experience working with patients in critical conditions
 BSN is strongly preferred
REQUIRED LICENSE/REGISTRATION/CERTIFICATION:
 Current licensure as a Registered Nurse in the State of Kansas
