Position Overview:
Greenlife Healthcare Staffing is seeking a motivated Nurse Practitioner (NP) Cardiology or Vascular to join a dynamic multi-specialty practice in the Bronx, NY. This role is open to experienced NPs, offering the chance to provide high-quality cardiac care in a collaborative, innovative environment.

Key Responsibilities:
Provide direct patient care across the care continuum, including performing histories and physical exams, diagnosing and managing acute and chronic conditions (e.g., heart failure, coronary artery disease, hypertension, peripheral arterial disease), and ordering/interpreting diagnostic tests (e.g., ECGs, echocardiograms, vascular studies).
Perform specialty-specific procedures (e.g., wound debridement, suture removal, cardioversion assistance, vascular access management) and develop, implement, and adjust evidence-based treatment plans, including medication prescription and management.
Serve as a pivotal point of contact, coordinating care with physicians, surgeons, rehabilitation staff, and other healthcare providers. Facilitate patient education on disease management, lifestyle modification, and medication adherence, and ensure smooth transitions between inpatient and outpatient settings.
Educate patients and their families on disease processes, treatment options, risk factor modification (diet, exercise, smoking cessation), and self-management strategies to promote optimal health outcomes and prevent disease progression.
Maintain accurate and timely electronic health records (EHR), document patient encounters, and manage long-term follow-up care, including monitoring treatment efficacy, managing complications, and adjusting care plans as needed.
