The Department of Medicine, Division of General Internal Medicine is seeking a Nurse Practitioner (NP) for the outpatient medicine clinic setting to help co-manage a small panel of high-risk adult patients in a primary care practice, enhancing standard PCP care. The NP will collaborate with established resident and faculty PCPs in the Johns Hopkins Outpatient Center Medical Clinic to provide enhanced, high touch, independent and collaborative primary care to a select group of high-risk patients. The NP will have extended visit times and significantly lower visit volumes than seen in standard primary care practices in order to develop meaningful relationships with patients at-risk for hospitalization. The NP will collaborate and coordinate care with interdisciplinary care teams to ensure the plan of care is in line with and supports the patient’s goals of care, physical and social needs. The NP will be responsible for documentation of visits and follow up re-assessment(s) in the electronic medical record, along with recommendations and/or prescriptions.

Starting Salary Range: $100,000 - $152,900 Annually ($113,000 targeted; Commensurate w/exp.)
