Nurse Practitioner / Physician Assistant – Outpatient Palliative Care


Location : Ocean County, New Jersey


Position Overview


We are seeking a compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join a growing outpatient palliative care team serving patients across skilled nursing facilities (SNFs), community settings, and home-based care.


This is an excellent opportunity for providers who are passionate about caring for patients with serious illnesses. Previous palliative care experience is preferred but not required. Candidates with experience in oncology, critical care, hospital medicine, geriatrics, skilled nursing, long-term care, primary care, or home-based care are encouraged to apply.


Schedule




  • Full-time




  • Primarily Monday–Friday




  • Flexible scheduling available




Responsibilities




  • Provide outpatient palliative care consultations and follow-up visits in skilled nursing facilities and community settings.




  • Assess and manage complex symptoms for patients with advanced illnesses and chronic conditions.




  • Conduct goals-of-care discussions and advance care planning with patients and their families.




  • Collaborate with physicians, nurses, social workers, care coordinators, hospice providers, and other healthcare professionals.




  • Support care transitions, including hospital discharges, skilled nursing stays, home care, hospice referrals, and long-term care planning.




  • Maintain accurate and timely clinical documentation.




  • Travel locally within the assigned service area.




  • Deliver compassionate, evidence-based, patient-centered care.




Qualifications




  • Active New Jersey Nurse Practitioner or Physician Assistant license.




  • National board certification.




  • DEA and CDS registration, or ability to obtain before start date.




  • Two or more years of clinical experience preferred.




  • Previous palliative care experience is preferred but not required.




  • Experience in oncology, critical care, geriatrics, hospital medicine, skilled nursing, long-term care, primary care, or home-based care is highly valued.




  • Palliative care certification (ACHPN, CHPN, or equivalent) is a plus.




  • Excellent communication, clinical judgment, and interpersonal skills.




  • Ability to work effectively within an interdisciplinary team.
