Nurse Practitioner- Primary Care

Summary: Our client is seeking a experienced and
compassionate Nurse Practitioner or Physician
Assistant to join their team in delivering high-quality care to our diverse
and underserved patient population. The ideal candidate will diagnose, educate,
and initiate treatment in an outpatient clinic setting, collaborating with a
multi-disciplinary team to promote health and prevent disease. This position
offers the opportunity to make a meaningful impact in a health center committed
to comprehensive, patient-centered care.

Core Responsibilities:

• Comprehensive
  Clinical Care: Provide comprehensive primary care services to patients
  of all backgrounds, including preventive, wellness, and acute care visits.
  Diagnose, treat, and manage acute and chronic conditions using
  evidence-based guidelines.
  
• Team-Based
  Care & Coordination: Collaborate with nurses, care coordinators,
  behavioral health providers, and specialists to manage care transitions,
  ensuring continuity, particularly for medically complex patients.
  
• Patient
  Advocacy & Communication: Build trusting, long-term relationships
  through effective, empathetic communication. Educate patients and families
  on disease management and healthy lifestyle changes.
  
• Efficient
  Workflow & Technology Adoption: Utilize Electronic Health Record
  (EHR) systems to maintain accurate, up-to-date, and detailed documentation
  in the electronic medical record system.
  

• Setting: 100% Outpatient Primary Care
  

• Patient Base: Senior/ Adult Focused
  

• Schedule: Monday–Friday
  

• Care Team: Comprehensive support (Nurses, Care Coordinators)
  

• EMR: Athena
  

• Core Focus: Preventive care, chronic disease management, and long-term patient continuity
  

Requirements

Qualifications:

• Graduate of an accredited NP or PA program
  
• Current unrestricted APRN or PA license
  
• National board certification & BLS certification
  
• DEA registration​
  
• Interest in value-based care and population health
