Job Description - Nursing Practice & Training Specialist, VNA of Cape Cod
Assist the Team Manager in the planning, development and provision of staff education programs.
Provide ongoing evaluation of the quality and effectiveness of educational activities to ensure and enhance professional development that promotes the delivery of safe, high-quality care.
Provide EPIC training for new hires during orientation, clearly relaying expectations of documentation and serving as a resource for program education
Participate in staff performance review process by conducting co-visits to ensure and enhance staff's ability to develop patient-centered plans of care with consistent implementation of and adherence to best evidence-based practices.
Provide written feedback to clinicians regarding clinical and documentation skills.
Conduct chart audits to ensure value-based measures are being met and identify opportunities to mitigate challenges.
Work closely with members of the education team to provide ongoing support to field clinicians.
Provide nursing clinical expertise to Team Leaders and non-clinical staff.
Serve as a mentor/preceptor and immediate resource for new clinical staff during initial four months of orientation.
Focus on areas of staff competency and training needs as delineated by HHCAPS, DPH, accrediting bodies, and conditions of participation.
Assess individual employee performance during orientation, retraining phase and during acquisition of new skills.
Communicate staff performance issues to appropriate Team Leader.
Provide leadership as both a clinical education resource and professional role model using sound knowledge, mentoring and coaching skills.
Continually seek out relevant training material and courses to enhance staff effectiveness and knowledge.
Work closely with Supplies to ensure that proper supplies are being ordered and delivered for appropriate wound care
Conduct wound consults for clinicians in the field, discuss recommendations with clinician and maintain positive relationships with wound care physicians
Responsible to present assigned materials during team meetings and act as a resource for clinicians
Consistently provides service excellence to all patients and co-workers in a manner that reflects VNA’s commitment to our mission.

Graduate of an accredited RN Program
Current Registered Nurse license in Massachusetts
Experience in Wound/ Ostomy Care with Wound Care Certification (WCC) or Wound Ostomy Certification (WOCN) preferred
IV Experience Preferred
5 years Nursing experience; with 2 years in Home Health.
