Job Summary: 

Responsible for planning and performing high quality Occupational Therapy for the patients referred to Witham Memorial Hospital Occupational Therapy Department and its contractual clients.

Minimum Requirements: 

• Bachelors, Masters or Doctorate degree from an accredited Occupational Therapy Program.


• Current certification or certification eligible by the National Board of Certification of Occupational Therapy (NBCOT).


• Current license and in good standing by the State of Indiana or temporary license for new graduates.

Upon employment and yearly thereafter, must complete mandatory inservices. Must be knowledgeable of disaster and safety procedures. Will review the Rehab Services policy and procedure manual every two (2) years. Shall be CPR certified in Basic Life Support, Course "C".
