SUMMARY


Provides Occupational Therapy (OT) services including testing, assessment, individual treatment, treatment planning, and monitoring individuals referred, as appropriate.  Participates in planning and implementing curriculums and procedures and acts as a member of the interdisciplinary treatment team. Acts as a consultant for staff, residential providers, caretakers, and families in implementing treatment and training.


ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:



  • Provides assessment as needed.

  • Assessment includes the following procedures:  clinical interviews, direct observation, formal testing, social/adaptive assessment, record review, written report, and recommendation as to treatment, training services, placement, etc., as appropriate and/or requested.

  • Written assessment reports will generally include identification of presenting problem; diagnosis; pertinent history; strengths and needs; adaptive functioning; maladaptive behaviors with antecedents; recommendations as to treatment/training; potential for less restrictive environments and interventions; barriers to attainment of goals/progress.

  • When a different or specific format is required by certifying or funding agency standards (e.g. OBRA Level II assessments) that format will be utilized.

  • Attends consumer meetings and participates as a member of an interdisciplinary treatment team to assess, develop, and monitor the effectiveness of treatment, support, and training plans for persons assigned.

  • Develops and prioritizes training and treatment plans, according to strengths and weaknesses.

  • Develops procedures as needed and documents progress as required by relevant standards of the agency and certifying/funding bodies.

  • Acts as a consultant to Supports Coordinators/Case Managers, Skill building, and other agency staff, as well as families and caretakers, regarding individuals served.

  • Consultation services include direct observation; conferences with individuals, staff and caretakers; baseline procedures; goal/plan writing and monitoring; documentation of services and progress/effectiveness; in-service on specific treatment strategies as well as general in-service on curriculums and OT techniques and interventions.

  • Provides direct OT services as needed including the development and monitoring of treatment plans in accordance with Medicaid and other applicable standards.

  • Complete OT evaluations for home modifications for consumers

  • Participates in committees as assigned to review, plan, and implement systems/interventions/procedures.

  • Complete equipment requisitions on as needed basis for consumers.

  • Provides training and in-service as needed to staff, caretakers and other individuals.

  • Develops functional knowledge of systems, procedures, forms, and timelines required by agency and certifying/funding bodies including DMH, Medicaid, OBRA, and CARF relative to job duties.

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute items such as range of motion, and to calculate percentages and interpret data in areas such as demonstrating measurable progress.


REASONING ABILITY:


Ability to assess and define problems, collect data, establish facts, draw valid conclusions, and draft effective interventions. Ability to interpret an extensive variety of information and instructions in various forms and deal with multiple variables.


COMPUTER SKILLS                           


To perform this job successfully, an individual should have knowledge of Word Processing software.

Possession of a valid Driver's license and ability to transport self and others in a motor vehicle.

Nationwide 401(a)-


If hired on or after 9/1/2019 the Authority shall contribute an amount equal to three percent (3%) of the employee’s base wage.


 


 


 


 


 


Pension-


For Employees hired prior to 09/01/2019, the pension formula will be a two percent (2%) multiplier and the Employer will continue to provide contributions as actuarially determined to insure the financial soundness of the fund.


 



 


 


Vested in the Authority’s contributions once thirty-six (36) months of service are completed with the Authority.
