Hospice Registered Nurse (RN)

As a Hospice Registered Nurse, you have the opportunity to make a meaningful difference during one of the most important and personal times in a patient and family’s life. You’ll provide compassionate, skilled nursing care to terminally ill patients while helping families navigate their journey with comfort, dignity, and understanding.

You’ll work closely with patients, families, physicians, and the interdisciplinary team to ensure each patient receives personalized care that honors their wishes and meets their unique needs.

JOB SUMMARY

The Hospice Registered Nurse provides compassionate, comprehensive nursing care to terminally ill patients in their homes and other care settings. This role focuses on comfort, quality of life, symptom management, patient dignity, and family support.

The RN serves as an important resource for patients and families, providing education, guidance, reassurance, and support throughout the hospice journey and into bereavement. The RN works collaboratively with the Hospice interdisciplinary team to coordinate care and ensure continuity across all settings.

QUALIFICATIONS

Education:

• Graduate of an accredited school of nursing.

Licensure:

• Current, unrestricted Registered Nurse license in the State of Ohio.

Experience:

• Minimum of 2 years of nursing experience preferred.


• Recent direct patient care experience within the past three years preferred.


• Hospice experience is a plus, but we are committed to providing the training and support needed to help the right candidate succeed.

• Ability to work independently while remaining connected to the Hospice team.


• Ability to make accurate and timely clinical decisions, including during urgent or unexpected situations.


• Ability to respond calmly and appropriately to patient and family needs in the home setting.


• Strong communication, teaching, organization, and interpersonal skills.


• Ability to supervise and provide direction to other members of the nursing team.


• Flexibility and respect for diverse social, cultural, racial, and religious backgrounds.


• Ability to successfully complete the Hospice training program.

WHAT YOU'LL DO

As a Hospice RN, you will:

• Regularly assess and reassess the physical, emotional, and nursing needs of each patient.


• Provide compassionate, skilled nursing care focused on comfort and quality of life.


• Perform treatments, symptom management, and nursing procedures according to physician orders and the patient’s plan of care.


• Monitor changes in the patient’s condition and communicate significant findings to the physician and interdisciplinary team.


• Educate and support patients and families, helping them feel confident and prepared to provide care.


• Develop, implement, and regularly evaluate individualized patient and family care plans in collaboration with the interdisciplinary team.


• Coordinate care for patients residing in SNF, NF, ICF, or MR settings.


• Participate in interdisciplinary team meetings and care conferences.


• Provide dietary guidance and other education appropriate to the patient’s needs.


• Supervise and assist with training other nursing service personnel as appropriate.


• Maintain accurate, timely, and complete clinical documentation and records.


• Protect patient privacy and maintain confidentiality in accordance with HIPAA and Hospice policies.


• Communicate promptly with your supervisor when a patient’s needs cannot be met or a procedure cannot safely be performed.


• Participate in the Hospice on-call rotation and provide coverage as assigned.


• Assist with orientation, education, and training of team members as requested.


• Maintain current clinical knowledge, skills, and competencies.


• Follow all Hospice, infection control, safety, and regulatory policies and procedures.


• Effectively utilize technology, including iPads, laptops, MatrixCare, and other systems necessary to provide quality patient care.


• Manage your schedule and time effectively to meet patient needs and attend required meetings and team activities.


• Work collaboratively within an interdisciplinary model of care.


• Perform other duties as assigned by the Director.

PHYSICAL REQUIREMENTS

This position requires the ability to perform physical activities necessary to provide safe patient care, including walking, kneeling, squatting, reaching, twisting, climbing, and assisting with lifting and/or transferring patients of varying sizes. The position may involve exposure to changing temperatures and humidity levels.

The employee must possess adequate vision and hearing, with or without appropriate adaptive devices, to safely perform the essential functions of the position. Annual TB testing and other required health screenings must be maintained in accordance with applicable requirements.

MENTAL & EMOTIONAL REQUIREMENTS

Hospice nursing requires compassion, sound judgment, flexibility, and the ability to remain calm during challenging situations. The RN must be able to work independently, assess patient needs, prioritize care, and respond appropriately to changing circumstances.

The ability to communicate effectively, build trusting relationships, manage emotional stress, and demonstrate professionalism and emotional stability is essential.

WORKING CONDITIONS

The Hospice RN may work in a variety of patient care environments and must be comfortable with exposure to odors, blood, body fluids, excrement, infectious materials, hazardous substances, and varying environmental conditions.

TRANSPORTATION REQUIREMENTS

Because this position involves providing care in multiple patient locations, the employee must have:

• A current, valid driver’s license.


• Current automobile liability insurance.


• Reliable transportation.
