Home Care On-Call Scheduling Coordinator for our New Hampshire Offices

Basic Function

To provide On-Call support as independently as possible, mostly on weekends, on a per-diem basis. To ensure excellent Customer Service in accordance with the company mission. Responsible for scheduling and supporting Caregivers in coordination with client needs and responding to urgent situations in a timely and professional manner.

Responsibilities

• To handle urgent situations through communication, trouble-shooting, caregiver support and scheduling coverage.


• Answer all incoming calls, handle all voice mails that come in the non-emergency phone line and respond to visit alerts.


• Update EVV to keep accurate log in and log out times with notes in shifts when something is adjusted.


• Act as liaison between clients, families and others involved in client care, direct care workers and management.


• Advise family or client contact in the event of scheduling changes.


• Identify and recommend changes in procedures to management when necessary to improve the quality of services.


• Keep current on latest technology to improve business.


• Thoroughly and accurately report all weekend activity in On Call format provided.


• Utilize Back-Up On Call Supervisor appropriately.


• Perform other related duties as assigned.
