Are you a compassionate Registered Nurse with oncology experience who is passionate about guiding patients through one of the most challenging journeys of their lives? We're seeking an Oncology Nurse Navigator (RN) to provide personalized support, education, and care coordination for oncology patients from diagnosis through treatment, survivorship, and beyond.
In this highly collaborative role, you'll serve as a patient advocate, helping individuals and their families navigate the healthcare system, overcome barriers to care, and ensure timely access to quality treatment and supportive resources.
What You'll Do
As an Oncology Nurse Navigator, you'll be a vital member of the multidisciplinary oncology team, coordinating care and improving the patient experience throughout the cancer care continuum.
Responsibilities
Serve as the primary advocate and resource for oncology patients and their families throughout diagnosis, treatment, and survivorship.
Educate patients on their diagnosis, treatment options, available resources, and care plans to support informed decision-making.
Coordinate care between physicians, clinic nurses, specialists, caregivers, and community resources to ensure seamless transitions across the continuum of care.
Identify and remove barriers to treatment by collaborating with internal and external healthcare providers.
Follow up with patients after surgeries, hospitalizations, emergency department visits, chemotherapy treatments, and other significant care events.
Facilitate communication among physicians, referring providers, patients, and multidisciplinary care teams.
Coordinate hospice, home health, skilled nursing, survivorship, and other supportive care services as appropriate.
Manage patient documentation, care plans, oncology histories, worklists, symptom management, and critical results using the electronic medical record.
Assist with quality improvement initiatives, accreditation activities, data collection, and departmental reporting.
Support policy and procedure development while ensuring compliance with regulatory and oncology practice standards.
Participate in staff education, professional development, and ongoing competency development within oncology nursing.
Maintain strict patient confidentiality and uphold HIPAA standards.
Qualifications
Current Registered Nurse (RN) license in New Hampshire
Graduate of an accredited nursing program
Minimum of two years of clinical nursing experience
Current Basic Life Support (BLS) certification
Strong communication, organizational, and critical thinking skills
Ability to effectively coordinate care across multidisciplinary teams
Compassionate, patient-centered approach with excellent interpersonal skills
Preferred
Previous oncology nursing experience
Experience with care coordination or nurse navigation
Experience working in a multidisciplinary oncology setting
Familiarity with electronic medical records, preferably MediTech
Oncology Nursing Certification (or willingness to obtain)
Certification Requirement
Oncology Nursing Certification Corporation (ONCC) certification must be obtained within 12 months of hire.
