The Outreach Program Coordinator creates, organizes and schedules outreach activities and special events. Engage patients and initiates care management services. This individual creates awareness and promote engagement, education and specific services to the community. Initiates and develops key relationships between individuals or groups in the community and the Mount Sinai Health System.
1. Develop,
implement, and maintain current outreach and retention strategy to identify and
target occupational health needs of various groups of patients in the
community as identified by needs assessment reports
2. Cultivate
strong working relationships by maintaining regular communication with
community and volunteer organizations to identify and utilize innovative
strategies to engage members and promote services offered by Mount Sinai
Health System facilities.
3. Develop,
implement and evaluate retention activities to help maintain and bolster
patient volume through focus groups, enrollment assistance and benefits counseling
sessions
4. Assist
in gauging program effectiveness to improve and support program operations and
standards, streamline work process and to increase the efficiency of the
program
5. Educate
and provide general assistance to various groups of patients regarding services
offered and other public benefits
6. Coordinate
and attend regularly scheduled meetings with outreach and retention
stakeholders
7. Act
as a liaison to advisory board members
8. Conduct
presentations at community meetings regarding various services offered
9. Coordinate
and plan seminars, conferences and other educational workshops
10. Assist in development of
marketing materials to support outreach and retention activities and implement
strategies that include but are not limited to website development,
advertisement, radio and printed media
11. Submit regularly activities
reports for funding agencies to meet reporting deadlines
12. Maintain meeting minutes and
other administrative duties as needed
13. Perform additional tasks as
assigned by the Manager of the Member Services Department
14. Performs other duties as
assigned.
Education Requirements
Bachelor’s degree. Master’s preferred or an equivalent combination of education and experience.

The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $66,482.07 - $74,250.00 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
