Under direction from the Central Intake and Referral Coordinator, responsible for conducting outreach for the Help Me Grow System of Supports, which include ASQ Sparkler, Family Connects, Home Visiting, Early Intervention and Community Resources in Northwest Ohio as part of the statewide central intake and referral system. 

Works to establish and maintain working relationships in the region that support central intake’s communication and outreach plan. Implement activities to recruit, engage and retain program participants, either directly or through referral sources. Travels throughout the region to perform assigned duties, in coordination with the Central Intake and Referral Coordinator. Assists with local, multi-county and Statewide events to increase public awareness and conduct child find. Tracks all outreach activities for monthly reporting requirements. Interact professionally with families, referral sources, and community stakeholders using strong interpersonal skills. 



Responsibilities

ESSENTIAL JOB FUNCTIONS

  1. Establishes and maintains relationships with home visiting and early intervention contractors in Northwest Ohio to identify referral sources and program options available in each county and assess outreach needs.

  2. Establishes and maintains relationships with primary referral sources in Northwest Ohio to educate them about program options and changes and generate timely and appropriate referrals for home visiting and early intervention services.

  3. Support and attend outreach events carried out by local and multi-county partners to generate home visiting and early intervention referrals.           

  4. Ensures the availability of marketing materials for outreach events and mailings through assembly and/or ordering of materials.

  5. Helps maintain home visiting waiting list by contacting assigned families to see if they are still interested in services and following up.

  6. Maintains regular communication with supervisor about daily schedule and activities while off-site and participates in all scheduled supervision and team meetings on-site.

  7. Track daily outreach activities for internal monitoring and monthly reporting to Bright Beginnings and other stakeholders.

  8. Identify any potential problem areas and report to Central Intake & Referral Coordinator.

 



Qualifications

MINIMUM QUALIFICATIONS:

  • High School Diploma or GED and four years of supervised experience in Customer Service, Social Service or Human and Family Development fields; OR holds an associate degree, from an accredited college or university in any field AND has at least two years of supervised, paid experience in case management. 

  • Must have a valid Driver’s License with driving privileges in the State of Ohio and insurable driving record.

  • Must complete background checks including FBI/BCI.
