Aegis Home Health & Hospice is seeking a motivated and relationship-focused Patient Care Navigator/Healthcare Marketer to join our growing team. This position plays a vital role in connecting patients, families, and referral partners with compassionate home health and hospice services throughout the community.

Position Summary


As a Patient Care Navigator/Healthcare Marketer, you will build and maintain referral relationships within the healthcare community while educating patients, families, and providers about Aegis Home Health & Hospice services. You will collaborate closely with clinical teams to support smooth patient transitions into care and serve as a trusted liaison between referral sources, patients, and caregivers.


This role also includes community outreach, networking, and helping expand awareness of Aegis services throughout the Omaha and Lincoln markets.


Qualifications


Qualified candidates should have at least two years of healthcare marketing, referral development, or relationship-building experience. Previous experience in hospice, palliative care, or home health is strongly preferred. Candidates must possess excellent communication and organizational skills and be self-motivated and goal-driven.


A valid driver's license, reliable transportation, auto insurance, CPR certification, and a negative TB test (or willingness to obtain) are required.
