Job Description - Patient Care Unit Coordinator (PUC) Full-Time

Description

The Patient Care Unit Coordinator assists the RN Clinical Coordinator(s) in providing direction and clinical support for a specified unit and assigned staff. This position performs administrative support activities required for proper transcription of medication, treatment orders, documenting and coordinating patient care. The Patient Care Unit Coordinator will maintain clinical records of the hospice patients and will function as a liaison between other team members assisting with day-to-day management duties.

Accountability: Reports to the RN Clinical Coordinator(s), and the Director of Nursing

FLSA Status: Non-Exempt/Hourly

Requirements

Essential Job Functions:

  1. Fosters and maintains an effective working relationship between co-workers, patients, and other professional members of the community
  2. Assures patient records are complete and all required documentation is filed in a timely manner
  3. Enters patient data and visits into the EMR system
  4. Responsible for faxing paperwork required by facilities
  5. Manages referrals for third parties
  6. Sends updates on Veteran patients to the VA
  7. Obtains records and responds to record requests as needed
  8. Assists with scheduling and staffing as requested
  9. Assists with closing open visits in the EMR system for billing
  10. Prepares documentation and assists with the interdisciplinary team meetings as directed
  11. Manages and processes referrals as needed, including obtaining provider signatures
  12. Obtains and responds tor requests for records
  13. May be responsible for running pharmacy reports for patients’ recertification
  14. Fills in on the switchboard for lunches, absences and time off as requested
  15. Posts on-call schedule daily
  16. May assist with making bereavement cards
  17. Other duties as assigned

  3. Adheres to departmental dress code requirements related to uniform, personal grooming, and other attire
  4. Participates in Performance Improvement activities as requested
  5. Demonstrates reliability by making every attempt to report to work on time, providing proper notification for absence or tardiness and follows company policies related to time off requests
  6. Demonstrates effective time management skills by coordinating activities to achieve maximum 
  7. productivity and efficiency, willingness to adjust or revise work schedules as requested
  8. Assumes responsibility for on-going personal development and continuing education
  9. Seeks guidance from a higher authority as needed

Physical/Safety Requirements:

  1. Must have the strength and endurance required for standing, walking, lifting, bending, pushing and pulling on a consistent basis up to 20 lbs.
  2. Utilizes appropriate durable medical equipment as needed in a safe manner
  3. Uses appropriate body mechanics 
  4. Position requires extended periods of sitting
  5. Persons in this position may be subject to potential exposure of infectious waste, disease, and other conditions, including TB, HIV and Hepatitis B
  6. Utilizes appropriate PPE as needed
  7. Follows the designated plan of action in the event of a fire or other emergency/disaster
  8. While performing the duties of this job, the employee is regularly exposed to fumes or airborne particles, work-related stresses, and driving hazards 
  9. An individual in this position may be required to drive to meetings with clients or health care professionals, as well as patients' homes. 
  10. The noise level in the work environment is usually minimal

Qualifications and Experience:

  1. Two years’ experience as a Licensed Practical Nurse, Medical Assistant or Certified Nursing Assistant in hospice, home health or geriatric care preferred
  2. One year of experience in an office setting, medical office experience required
  3. Demonstrated expertise with Microsoft and/or Google Suite applications
  4. Excellent interpersonal and communication skills
  5. Ability to multi-task as needed to meet deadlines
  6. Must have excellent attention to detail
  7. Successful completion of a background check pursuant to NRS 449.188 and NRS 179A.100 along with a pre-employment drug screen and physical, TB testing, previous employment and reference checking, and an MVR (may include verification of education)
  8. CPR/First Aid certified
  9. Ability to function with limited supervision and accountability to meet assigned projects and timelines
  10. Must be able to handle frequent interruptions
  11. Must be able to cope with emotionally charged situations encompassing, patients, families, and other caregivers
  12. Must have reliable transportation
