Job Description - Patient Navigator - Cancer Center - 8-Hour - Full Time - Days
Description In collaboration with the patient care team, the Patient Navigator works to remove the patient's barriers to care by identifying critical resources, helping navigate through health care services and systems, and promoting patient health. The Patient Navigator operates with a high level of autonomy providing care coordination and workflow processing as well as communication to the patient at all levels throughout their care, at the direction of the care team. The Patient Navigator will leverage understanding of specific practice specialty, physician and clinic needs to ensure all applicable documentation, verifications, authorizations and equipment/supplies are attained to effectively serve the patient needs. Pro-actively facilitates the process and effective communication on all levels through tactful and professional interaction. Uses knowledge of the clinic to resolve complexed issues and identify ways to increase patient satisfaction and outcomes. Provides advance level of non-clinical and administrative support to the staff, physicians, and patients. The Patient Navigator will be the subject matter expert and provide training to others and conducting training session(s) on new processes/workflows. Serves as an ambassador, change agent, quality champion, and mentor/resource.
Responsibilities
Serves as the point of contact and liaison for the patients and their care teams by assisting patients in accessing services within the organization, community and hospital based programs; facilitating communication with referring physicians, referring patients to appropriate resources, and providing follow-up.
Responsible for identifying concerns throughout treatment and facilitates timely the communication and collaboration between care teams, patients, and their families for problem resolution and to keep patients engaged in their care.
Analyzes logistics and make recommendations for barriers that would prevent a patient from arriving to their appointment or receiving services such as scheduling complications, transportation, cleaning and maintenance of equipment/supplies.
Responsible for obtaining all required documentation needed by Physician(s) for clinic appointments. Proactively and independently track all new and follow up patients. Takes lead on facilitating relevant insurance concerns or issues by connecting patients with appropriate resources and team members.
Responsible for facilitating resolution to the complex and difficult non-clinical and/or administrative issues and concerns within the clinic. Makes recommendations for improvement. Performs unit office procedures including all administrative duties as needed, such as scanning, filing, faxing, phones, and/or courier duties.
Monitors that all appointments have been scheduled including all ancillary procedures and interdepartmental services; coordinates new patient referrals and consultations with the physicians.
Proactively collaborates with members of the care team in coordinating and implementing plans for patient care and records patient history for medical staff review.
Supports clinic projects/program, organizational work, and/or councils or committees that will assist with improvement of patients’ healthcare outcomes. May be responsible for program data maintenance and tracking.
Assists with the development and implementation clinical quality assurance protocols and programs.
Monitors and maintains timely patient flow and room availability. Assists patients with the completion of forms, as needed.
May transport stable, non-acute patients for testing or treatment.
Qualifications
Education
High School Diploma/GED - minimum
Vocational/Technical Diploma Graduate of an accredited medical assistant program - preferred
Bachelor's Degree In a healthcare-related field - preferred
Experience
3 years Support experience for a Medical office, ambulatory care facility or medical group practice - minimum
3 years Experience providing a high level of customer service in a fast paced environment, including handling confidential data with discretion. - minimum
3 years Expertise in insurance contracts and payor requirements. - preferred
Licenses and Certifications
Basic Life Support (BLS) by American Heart Association or American Red Cross
Phlebotomy Certification CA - preferred CPT1
Certified Registered Medical Assistant - preferred
