Job Description - Patient Navigator RN - Emergency Department
Delta Health is seeking an experienced Registered Nurse to serve as a Patient Navigator in the Emergency Department. This role provides a combination of clinical, quality improvement, operational, educational, and administrative support to promote safe, efficient, and high-quality patient care.
The Patient Navigator will work closely with the Emergency Department Director, physicians, nursing staff, Trauma Program leadership, hospital departments, and community partners. The position may provide direct Emergency Department nursing care based on patient volume, acuity, staffing needs, and department priorities.
When not providing direct patient care, the Patient Navigator will support quality initiatives, data tracking, patient flow, staff education, policy and procedure updates, departmental projects, and other operational needs.
Key Responsibilities
Provide direct Emergency Department nursing care as needed and serve as a clinical resource to ED staff.
Support safe and efficient patient flow, admissions, transfers, and patient disposition.
Assist with quality and performance improvement initiatives, including data collection, chart audits, tracking, and follow-up.
Support the COPPER program and pediatric emergency care quality initiatives.
Identify trends and opportunities to improve patient safety, outcomes, efficiency, and the patient experience.
Assist with development, review, and implementation of ED policies, procedures, workflows, and educational materials.
Coordinate or assist with staff education, competencies, skills reviews, and orientation.
Support departmental data collection, reporting, dashboards, meetings, and quality reviews.
Assist with scheduling, payroll review, project tracking, and other administrative functions as assigned.
Support regulatory and accreditation readiness, including audits, surveys, and corrective action activities.
Collaborate with physicians, nursing staff, EMS, hospital departments, and community healthcare partners.
Promote a culture of teamwork, accountability, professionalism, patient safety, and continuous improvement.
Required Qualifications
Current Colorado RN license or multistate compact RN license.
Emergency Department nursing experience required; 2 years of ED or critical care experience preferred.
Current BLS certification.
ACLS, PALS/ENPC, and TNCC required within established department timeframes.
Strong communication, organization, analytical, and problem-solving skills.
Ability to manage multiple priorities independently while working effectively with a multidisciplinary team.
Experience with electronic health records, data tracking, quality improvement, education, or departmental operations preferred.
Proficiency with computer systems and Microsoft Office preferred.
