Pediatric Multispecialty Navigator

We are looking for dedicated Pediatric Multispecialty Navigator in the Fairfax area to work in children’s specialties including Inova Children's Hematology/Oncology, Inova Children's Pulmonology, Inova Children’s Gastroenterology, Inova Children’s Neurology and Inova Children’s Orthopedics & Sports Medicine. This role will be full-time, day shift with varying hours from 7:30 a.m. – 5:30 p.m. Monday – Friday.

Pediatric Multispecialty Navigator

The Pediatric Multispecialty Navigator acts as a liaison, advocate, and point of contact for parents and caregivers of pediatric patients by helping them access the best healthcare outcomes and critical resources. In addition to providing emotional support to parents and caregivers, this position guides the family through the health system from diagnosis, testing, treatment, and follow-up care including social work, caretaker education and transportation to assist patients with navigating the continuum of care. May assist patients and families with resolving financial, psychosocial, functional, and administrative issues by advising of options and referring to appropriate resources. Eliminates barriers for parent’s or caregiver’s access to the child’s providers and facilitates continuity of care/care coordination. Acts as advisor/educator by providing emotional support, counseling, clinical education, resources, and expert guidance to parents and caregivers to promote their ability to understand and meaningfully participate in the healthcare process for their child including advocating for options and personal decision-making in the care plan. Provides ongoing collaboration with specialty areas including, but not limited to oncology, surgery, allergy, diabetes educators, and social work in the care of one pediatric patient. This position builds working relationships with parents and caregivers, healthcare providers, and community resources to maximize patient outcomes and identify/resolve barriers to the plan of care. Consistently seeks to improve healthcare experience and positive patient outcomes.

Duties and Responsibilities:

• Coordinates a patient's plan of care which may include appointment scheduling, confirming parents’ and caregivers’ attendance, transportation, assisting with referral process, and answering parent’s and caregiver’s questions.
• Provides educational information and advocacy to the parents and caregivers throughout the care process in accordance with service line policies and procedures.
• Functions as a single point of contact to internal and external community/family services for parents and caregivers in need of services such as screening, diagnostic treatment, clinical research participation, support services, and follow-up/aftercare plan.
• Assists families with resolving financial, psychosocial, functional, and administrative issues by advising of options and referring to appropriate resources.
• Develops and maintains reliable systems to document, track, and monitor patient data.
• Demonstrates proficiency with caseload assignment and ability to manage complex cases effectively.
• Uses specialty background to provide patient care and resources specific to pediatric diseases and conditions.
• Educates family, caregivers, and members of the healthcare delivery team regarding treatment options, community resources, insurance benefits, self-care management, etc. to facilitate timely and informed decision-making throughout all phases of the continuum process.
• Encourages and coaches parents and caregivers to be engaged in their child’s care planning and to embrace their essential role in the decision-making process.
• Collaborates with a multidisciplinary team to address the parents’ and caregivers' questions and responses about the care process and treatment options. Discusses payor criteria and issues on a case-by-case basis with clinical staff and follows up to resolve problems with payors as needed.
• Ensures satisfaction with the integrated health care delivery model and access to appropriate care or facility.
• Actively participates in obtaining quality outcomes such as decreasing times between referral, diagnosis, and treatment.
• Provides a personal, single point of contact for parents and caregivers, provides referrals to support and ancillary services as needed, and ensures that the family has appropriate access to health care services and providers.
• May perform additional duties as assigned.

Minimum Requirements:

• Education: Bachelor’s degree in Nursing or related Bachelor's Degree

• Experience: 3 years of progressive clinical experience

• Certification: Basic Life Support (BLS) upon start; If the Team Member is a Diabetes professional, must have CDE or certification specific to the specialty

• Licensure: Registered Nurse is required if the Team Member has a BSN. If Team Member has BSN, they must also have an RN license and be eligible to practice in VA

Preferred Qualifications:

• Previous Pediatric Specialists of Virginia (PSV) work experience
