Peer Support Specialist (Homeless Services)

Responsibilities: 

• Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards; in addition, documentation complies with patients' insurance requirements. 

• Engages patients in meeting their healthcare needs and cultivates patient connectedness with the clinic and community. Participates in care coordination. 

• Leads and/or co-leads peer recovery groups. 

• Provides engagement, assistance and support to patients, encouraging healthy activities and compliance. 

• Utilizes supervision and reports any high risk situations. 

• Perform other duties as assigned. 

Requirements:

• 2 years personal experience as a mental health services consumer.
• Bilingual in English/Spanish required.
• Completion of Peer Employment Training.
• High school diploma or GED equivalent.
• Or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements.
• Traveling between sites and other locations is required as an essential function of the job. Must have a car, a valid California driver’s license, and proof of minimum levels of car insurance as required under California law, although limits of $100,000 are recommended. An acceptable driving record is also required. California law requires all drivers to obtain a valid California driver’s license within ten days of establishing residency. Mileage and other reimbursement governed by policy.
• Ability to maintain good interpersonal interactions with clients and co-workers as a member of a multi-disciplinary community health team with a diverse multi-cultural population.
• Basic computer literacy ability to comply with department needs and expectations (i.e., electronic medical record documentation, obtaining background information and reports on patients, following up on appointments, etc.).
• Demonstrated ability to maintain good working relationships with employees, coworkers, and departments.
• Demonstrated social skills, limit-setting, and boundaries.*Intermediate interpersonal and customer service skills
• Intermediate written and verbal communication skills, including ability to communicate effectively with patients and family members who speak English as a second language.
