The Role
What You’ll Do
- Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
- Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
- Data into action: Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
- Close the loop: Complete documentation/charts within the same day
- High-volume rounding: Efficiently complete visits across designated SNFs (typ. 25+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.
- Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting
- Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.
- Regional flexibility: Cover sites within a 30–40-mile radius; for sites outside of that radius, mileage and lodging covered per policy.
How Success is Measured (KPIs)
- Visits: Target 25 patient visits/day
- Engagement: ≥80% Time to Acknowledgement Rate: <24h
- Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.
- Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.
Must-Have Qualifications
- Certified and licensed Nurse Practitioner in ME; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
- Eligible for Medicare participation; company bills on your behalf.
- 2+ years NP experience with complex adult populations; able to practice independently.
- Valid ME driver’s license; comfortable traveling locally.
- Thrives in a dynamic, evolving startup environment.
- English required
Nice-to-Haves
- Tech-savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
- Spanish is preferred.
Compensation & Perks
- Per Diem, 16 to 24 expected hours per week
- Paid per visit model
- Conversion Rate Per Hour: ~$104-130
