Per Diem Registered Nurse (RN)- Home Care

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our team on a PRN/Part-Time basis. The ideal candidate will have a strong clinical background and a commitment to delivering high-quality, patient-centered care. This role involves working closely with patients, families, and caregivers in a home health setting to ensure optimal health outcomes. The Registered Nurse will be responsible for conducting assessments, developing individualized care plans, and providing nursing support as needed.

Duties

• Conduct comprehensive assessments of clients' health status and needs in their homes.


• Develop, implement, and evaluate individualized care plans based on client assessments.


• Monitor clients’ conditions and report any significant changes.


• Provide education and support to clients and their families about health management and care plans.


• Collaborate with caregivers and the management team to ensure quality client care.


• Maintain accurate and up-to-date client records, ensuring compliance with company policies.


• Act as a resource, mentor, and trainer for caregivers, offering guidance and support.


• Promote safety and maintain high standards of care in home environments.

Skills

• Strong knowledge of home care protocols and best practices.


• Proficient in patient assessments and care plan development.


• Excellent communication and interpersonal skills for working with clients and families.


• Ability to work independently and adapt to various client needs.


• Strong organizational skills to manage tasks effectively in a flexible schedule.


• Familiarity with basic electronic record systems (experience with specific systems is a plus).
