Per Diem Registered Nurse (RN) - Home Health

Endurance Home Care is a fast-growing, technology-enabled provider of private, in-home care, licensed as a Health Care Services Firm in New Jersey. We serve clients across Connecticut, New Jersey and New York.

We’re looking for per diem RNs who want flexible, meaningful work. You’ll help make sure every client gets safe, high-quality care from our Certified Home Health Aides. You'll report to Endurance Home Care's Nurse Supervisor.

LOCATIONS

1.   North New Jersey: Union, Essex, Bergen, Passaic, Morris Counties

2.   South New Jersey: Monmouth, Ocean, Camden, Burlington, Gloucester and Atlantic Counties

WHAT YOU’LL DO

Initial Client Assessments

• Do in-home nursing assessments for new clients
• Write individualized care plans based on each client’s needs, goals and home environment
• Meet with clients and their families to explain the plan of care and answer questions
• Help match clients with the right caregiver

60-Day Supervisory Visits

• Visit active clients at least every 60 days to review their condition and update care plans
• Supervise and support our CHHAs and make sure care meets NJ Board of Nursing and Health Care Services Firm requirements

• Flag changes in condition and escalate concerns to the care team and families
• Document visits on time in our digital platform (mobile-friendly)

• Active New Jersey RN license (required)
• At least 1 year of clinical experience; home care, elder care or community health preferred
• Reliable transportation and a valid driver’s license
• Comfortable doing independent in-home visits and documenting electronically
• Warm, clear communicator with clients and families

• Initial Assessment / Start of Care: $125 per visit

• 60-Day Supervisory Visit: $65 per visit

• Mileage reimbursement for travel between same-day client visits

• Potential for Full Time opporunity
