Pharmacy Technician Care Manager

As a Pharmacy Technician – Care Manager, you will work closely with healthcare teams and patients to optimize outcomes and improve overall health.

Responsibilities (RE)

• Conduct medication reconciliation and resolve discrepancies with care teams
  
• Educate patients on medication use, side effects, and adherence strategies
  
• Monitor medication adherence and follow up with at -risk patients
  
• Support care transitions, including refills, prior authorizations, and delivery coordination
  
• Maintain accurate documentation in EHR systems
  
• Collaborate with pharmacists to identify drug interactions and improve care plans
  
• Ensure compliance with HIPAA and pharmacy regulations
  
• Participate in quality improvement initiatives
  

Requirements

Qualifications

• Certification: Current Pharmacy Technician License from a recognized certifying body (e.g., PTCB or NHA).
  
• Experience: Previous experience in a pharmacy setting, ideally with a focus on care management or chronic disease management.
  
• Knowledge: Strong understanding of pharmacology, medication therapy management (MTM), and patient -centered care practices.
  

Skills

• Technology Proficiency: Familiarity with EHR systems, pharmacy software, and telehealth tools.
  
• Communication: Excellent verbal and written communication skills for patient and provider interactions.
  
• Attention to Detail: Exceptional accuracy in documenting patient information and managing medication orders.
  
• Problem -Solving: A proactive approach to identifying and resolving medication -related issues.
