Pharmacy Technician Specialist Medication History Varied La Jolla

Required Education/Experience/Specialized Skills: 

• High School Diploma or equivalent
  
   

Required Certification/Registration: 

• California Pharmacy Technician License
• Certification by a national pharmacy technician certification board (CPhT) within 6 months of hire. 
• ASHP Medication History Training Program within 1 year of hire
• Minimum 2 years' pharmacy technician experience. 
  
   

Preferred Education/Experience/Specialized Skills/Certification: 

• CPhT and ASHP Medication History Certificate
• 5 years' experience
• Prior experience obtaining medication histories from patients/caregivers

This is a full time position with varied, 10-hour shifts at Scripps Memorial Hospital La Jolla. Shifts are 0700-1745, 0800-1845, 1000-2045, 1330-0012, with rotating weekends off.

Responsibilities include:

• Under the supervision of a pharmacist, obtains accurate and complete medication histories for patients admitted to the acute care facility. 

• This position supports clinical pharmacists and providers by obtaining, verifying, and documenting patients' medication histories through direct patient interviews, caregiver communication, and review of external sources such as pharmacy records and prescription databases. 

• The technician contributes to patient safety by minimizing medication discrepancies and supporting optimal pharmacotherapy decisions.
