Physical Therapist

Nursing Vault Plus Staffing is seeking a dedicated and professional Physical Therapist for a 14 -week contract assignment at Chapel Manor in Philadelphia. This role is ideal for a clinician who is passionate about geriatric rehabilitation and helping patients regain their independence in a skilled nursing environment.

Key Responsibilities

• Assessment: Conduct thorough initial evaluations of patient mobility, strength, and functional limitations.
  

• Plan of Care: Develop and implement individualized 14 -week treatment plans with measurable goals.
  

• Therapy Delivery: Administer manual therapy, therapeutic exercises, and gait training to improve patient outcomes.
  

• Collaboration: Work closely with the Chapel Manor interdisciplinary team (OTs, SLPs, and Nursing) to coordinate comprehensive care.
  

• Documentation: Maintain timely and accurate clinical records in compliance with facility and state standards.
  

• Education: Instruct patients and families on safe mobility techniques and home exercise programs.
