Job Description - Physical Therapist

Little Colorado Medical Center is seeking a compassionate and motivated Physical Therapist to provide evaluation, treatment, and rehabilitation services for patients in both inpatient and outpatient settings. The Physical Therapist will work closely with physicians, nursing staff, and interdisciplinary teams to help patients restore mobility, reduce pain, improve functional independence, and achieve optimal recovery outcomes in a rural community healthcare setting.

Key Responsibilities:

Evaluate patients and develop individualized treatment plans based on physician referrals and clinical findings

Provide physical therapy treatments for orthopedic, neurological, post-surgical, acute care, and chronic condition patients

Implement therapeutic exercises, gait training, balance training, mobility programs, and pain management techniques

Educate patients and families on treatment plans, home exercise programs, and injury prevention

Document evaluations, progress notes, and discharge summaries accurately and timely in the EMR system

Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate patient care

Monitor patient progress and modify treatment plans as appropriate

Maintain compliance with facility policies, state regulations, and professional practice standards

Assist with discharge planning and recommendations for continued rehabilitation needs

Qualifications:

Graduate of an accredited Physical Therapy program

Active Arizona Physical Therapist license or eligibility to obtain licensure

Current CPR/BLS certification preferred

Previous hospital, outpatient, or rural healthcare experience preferred

Strong communication, clinical assessment, and patient education skills
