Your Day-to-Day

• Conduct comprehensive physical assessments and review medical histories to evaluate patient conditions
• Develop and implement individualized treatment plans aligned with rehabilitation goals
• Provide therapeutic exercises, mobility training, and functional rehabilitation programs
• Educate patients and family members on recovery strategies, injury prevention, and wellness practices
• Monitor patient progress and adjust treatment plans to optimize outcomes
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate care
• Maintain accurate and timely patient documentation in compliance with regulatory requirements
• Support patients in achieving greater mobility, function, and overall well-being within their home environment

What We're Looking For

• Minimum of nine months of experience as a Physical Therapist
• Home health or community-based therapy experience preferred
• Master's degree in Physical Therapy from a CAPTE-accredited Physical Therapist education program
• Current Physical Therapist license in the state of practice
• Valid CPR certification maintained throughout employment
• Valid driver's license and ability to travel to patient locations
• Strong clinical assessment, communication, and patient education skills
• Commitment to delivering high-quality, compassionate care
