Physical Therapist (PT) - Sign-On Bonus ($2,500) - PRN/Per Visit - Home Health

Job Summary:

We are seeking a skilled Home Health -Physical Therapist (PT) - PRN - Per Visit - Home Health to join our team in Dallas, TX. The ideal candidate will have experience in providing physical therapy services to patients in their homes. As a Home Health - Physical Therapist (PT), you will be responsible for assessing patients' needs, developing treatment plans, and providing therapy services to help patients regain their mobility and independence.

Responsibilities:

• Conduct initial assessments of patients' physical abilities and develop treatment plans


• Provide therapy services to patients in their homes


• Monitor patients' progress and adjust treatment plans as needed


• Communicate with patients' physicians and other healthcare professionals to coordinate care


• Document patients' progress and maintain accurate records


• Provide education to patients and their families on how to manage their conditions and prevent further injury

Requirements:

• Current license to practice as a Physical Therapist in the state of Texas


• Minimum of 1 year of experience in home health


• Excellent communication and interpersonal skills


• Ability to work independently and as part of a team


• Strong problem-solving and critical thinking skills


• Valid driver's license and reliable transportation
