Full time or Part time
We are seeking a compassionate and motivated Physical Therapist to join our medical practice in Rockford, IL. Our ideal Physical Therapist has tact and skill in patient management, excellent communication skills, and enjoys working as a part of a comprehensive team to offer our patients the best care possible, that they need and deserve! Ideally, we are seeking a provider who has experience with treating a wide range of patient conditions. This position offers excellent work life balance with no weekend hours and we are flexible in our need and will accommodate a Physical Therapist seeking either full time or part time work.

Job Duties:

Consult with referring Physicians/Healthcare team regarding diagnosis as needed
Develop and implement physical therapy treatment programs based on each patient's particular needs
Assist patients with all phases of physical therapy treatment and techniques to include exercise instruction, modern modalities, and activities of daily living
Patient education on the benefits of PT, the recovery process and the challenges they will face, treatment plan maintenance, and home exercises
Documentation/progress notes- accurate and timely records, document treatments rendered in patient charts, Record patient conditions and responses to treatments in charts after each visit
Collaboration with colleagues and staff for comprehensive patient care

Requirements:

Graduation from an accredited Physical Therapy program
Physical Therapy license to practice in IL

Schedule:

Full time (Monday – Friday) Friday office hours end at 1 or Part time
No Weekends!

Salary:

$95k-$110k per year (depending on experience and training)
