PACE North is looking for a dedicated and compassionate Physical Therapist to lead and deliver high-quality care to the aging population - both at our Center and in participants' homes.

  

We are seeking a licensed Physical Therapist to join our team on a temporary, fixed-term basis to provide coverage during a maternity leave.

  

Position Details

  • Temporary /      Fixed-Term
  • Approximate      duration: 4 months
  • Schedule:  Full-Time, hours negotiable, Monday - Friday

  

What You’ll Do:

As a Physical Therapist at PACE North, you’ll be responsible for helping participants maximize their mobility and independence. You’ll:

  • Conduct initial and periodic assessments of participants’ physical and functional status
  • Develop personalized care and treatment plans, including therapeutic exercise, gait training, and mobility support
  •  Collaborate with an interdisciplinary care team and communicate changes in participant condition
  •  Recommend and evaluate the need for durable medical equipment
  • Train family members, caregivers, and staff on implementing therapy strategies
  •  Contribute to participant discharge planning and continuity of care across settings

Requirements

  

What We’re Looking For:

   Education  

  •   Bachelor’s in physical therapy, Master’s, or Doctorate

 Licensure/Certifications:

  •  Licensed Physical Therapist in Michigan
  •  CPR Certification (We can train)
  •  Valid Michigan Driver’s License

  Skills & Qualities:

  • Strong clinical competency in geriatric care
  •  Basic computer proficiency
  • Ability to work independently and collaboratively
  • Clear communication and documentation skills
  • Commitment to participant-centered care and quality improvement
