Bloom Healthcare is an employee-owned practice changing the way primary care is delivered to the most vulnerable patients.  The Physician role works as a lead in our team-based care environment. We are a value-based care provider focused on quality of care for the patients we serve. Our primary care team consists of doctors, advanced practice professionals, dedicated administrative and care management.   Additional Bloom resources include social work, pharmacy, behavioral health, population health support and integrated hospice. Our value-based care model provides higher quality care and better outcomes for seniors by providing a concierge experience, multidisciplinary services, coordinated care supported by analytics and tools, and deep community relationships. This approach allows our Physicians to see fewer patients per day and spend more time with those they do.

We understand that every patient has unique experiences and goals for their health, so we tailor each personal care plan based on these goals. Each of our clinicians has a passion for the interpersonal relationships created when caring for a patient in their home.

We are currently looking for a Physician to join our Team!

As a Physician with Bloom you’ll enjoy:

• Participation in our employee equity plan
• Base Salary of $225,000
• Quality Bonus of up to $35,000
• Spend more time with your patients practicing quality care
• No primary call responsibilities
• Empaneled patients with data-driven support
• A defined territory within reasonable proximity to your home address
• Flexible schedules (3,4,5-day work weeks)
• 10 visits per day

Responsibilities

• Deliver comprehensive primary care at home visits to 10 patients per day
• Provide data-driven, condition-specific specialty visits to patients as needed
• Manage the health outcomes of an empaneled patient population
• Coordinate patient care with team of APPs
• Demonstrate exceptional communication and relationship-building skills with care team members, including internal clinicians and external partners
• Engage with and appropriately utilize population health data to drive superior patient outcomes
• Attend ongoing educational opportunities
• Commute to assigned assisted living communities to provide care at home
