Position Overview:
New Horizons is seeking a Board-Certified Physician (MD or DO) to serve as a Specialized Healthcare Consultant (SHC) for our Healthcare Home Program. This is an ideal opportunity for a physician who enjoys care coordination, population health, and collaborative care and is seeking a flexible, part-time consulting role that complements an existing practice, retirement transition, or other professional commitments.
This is a consulting position only. No direct patient panel, no on-call responsibilities, and no prescribing required.

What You'll Do
As the SHC, you will provide medical expertise and consultation to the Healthcare Home team, helping improve health outcomes for adults living with behavioral health conditions.
Responsibilities include:
· Providing clinical consultation to ensure Healthcare Home services align with current medical standards of care
· Collaborating with Nurse Care Managers and a Care Coordinator regarding enrollees’ physical health needs
· Consulting with psychiatrists and other members of the integrated treatment team regarding health and wellness concerns
· Supporting coordination of care with primary care practices, specialists, hospitals, and community medical providers
· Participating in review of population health data and CareManager reports to identify priorities and intervention strategies
· Attending Risk Consultation meetings to provide recommendations for individuals with elevated health risks
· Assisting with staff education and training on physical health and wellness topics
· Supporting New Horizons’ mission of integrating physical and behavioral healthcare
About the Healthcare Home Program
The Healthcare Home Program provides coordinated, whole-person care for individuals with serious mental illness and chronic health conditions. The SHC works alongside Nurse Care Managers, Care Coordinators, psychiatrists, therapists, peer specialists, community support staff, and residential team members to address health disparities and improve physical health outcomes.

· MD or DO degree from an accredited medical school
· Board Certified in Family Medicine, Internal Medicine, or related primary care specialty preferred
· Current Missouri medical license in good standing
