Job Summary:

A premier post-acute medical group is seeking an autonomous Post-Acute Care Primary Care Physician (MD/DO) to lead facility-based care in Lake Wales, FL. This role is ideal for a clinician who enjoys the pace of transitional care, performing daily rounds across Skilled Nursing Facilities (SNFs). You will take the lead on managing high-acuity patients, welcoming new admissions, and providing structure and supervision to local Advanced Practice Providers (APPs).

Schedule:

• Full-time: 5 days per week
  

• Hours: Monday to Friday, 8:00 AM – 5:00 PM
  

Key Responsibilities:

• Perform daily rounds at designated Skilled Nursing Facilities (SNFs) in the Lake Wales area.
  

• Review, examine, and admit incoming new patients to the sub-acute units.
  

• Directly intervene and manage care for higher-risk or clinically declining patients.
  

• Supervise, guide, and sign off on charts for local APPs.
  

Qualifications & Requirements:

• Board Certified (BC) or Board Eligible in Internal Medicine or Family Medicine (Board Certification is NOT a requirement).
  

• Active Florida Medical License.
  

• ACN (Area of Critical Need) license holders are accepted and welcome to apply.
  

• Strong clinical decision-making skills in a post-acute setting.
  

Compensation & Benefits:

• Base Salary: $225,000 – $245,000 per year
  

• Time Off: 2 weeks (14 days) of Paid Time Off (PTO)
