Position Summary:
As an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will provide comprehensive primary care services within skilled nursing and senior living communities. This role includes routine rounding, proactive chronic disease management, coordination of care across interdisciplinary teams, and close collaboration with facility partners. Our value-based approach emphasizes preventive care, improved quality outcomes, and a better experience for patients, families, and care teams.
Qualifications:
Current NP or PA licensure in the state of practice
Previous professional patient care experience
Experience in geriatrics, family medicine, or internal medicine preferred.
Exceptional patient-centered service skills, including compassionate communication, active listening, professionalism, and the ability to build trusting relationships with residents, families, and facility staff
Strong collaboration skills with the ability to work effectively across interdisciplinary teams
