Position: Primary Care Physician – Family Medicine / Internal Medicine
Location: Anaheim, CA – Orange County

HealthPlus Staffing is seeking a Board Certified or Board Eligible Family Medicine or Internal Medicine Physician for a full-time opportunity in Anaheim, California.
The physician will provide comprehensive primary care to older adults within a PACE (Program of All-Inclusive Care for the Elderly) model. This position is ideal for a physician who enjoys managing complex medical needs while working closely with an interdisciplinary healthcare team.
The incoming physician will play an important role in assessing participants, managing acute and chronic conditions, developing individualized treatment plans, and coordinating care across multiple disciplines.

Comprehensive patient assessments and individualized care planning
Preventive medicine and chronic disease management
Coordination of care for medically complex patients
Collaboration with physicians, advanced practice providers, nursing, and other members of the interdisciplinary care team
Physician serves as a source of medical and geriatric expertise for the care team
Supervisory collaboration with Nurse Practitioners and/or Physician Assistants as applicable
Participation in quality-improvement initiatives and clinical care planning
Conduct initial and ongoing comprehensive medical assessments
Obtain medical histories and perform physical examinations
Diagnose and manage acute and chronic medical conditions
Order and interpret laboratory and diagnostic testing
Develop and update individualized treatment and care plans
Manage medications and adjust treatment when clinically appropriate
Provide preventive care, health screenings, and patient education
Evaluate changes in patients' physical, functional, and cognitive status
Coordinate specialty consultations and additional medical services when needed
Collaborate closely with the interdisciplinary care team
Educate patients and families regarding diagnoses, treatment plans, medications, and disease management
Coordinate care when assigned patients require hospitalization or skilled nursing care
Maintain accurate and timely clinical documentation
Participate in quality assessment and performance-improvement activities
Base Salary: $250,000 – $280,000 annually

Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
Board Certified or Board Eligible in Internal Medicine or Family Medicine
Must have an active California medical license
Must have an active DEA license
Must not be restricted from seeing Medi-Cal or Medicare patients
Minimum of one year of documented experience working with a frail or elderly population
Geriatric medicine experience or additional qualifications in Geriatrics preferred
Current CPR/BLS certification
Strong clinical assessment and chronic disease management skills
Ability to effectively manage medically complex older adults
Strong communication and interpersonal skills
Ability to work collaboratively within an interdisciplinary healthcare team
Commitment to compassionate, patient-centered care
The ideal candidate is a Family Medicine or Internal Medicine physician with an interest in caring for older adults and managing patients with multiple chronic conditions and complex healthcare needs.
The physician should value a collaborative approach to medicine and be comfortable working closely with an interdisciplinary team to coordinate comprehensive care for each participant.
