Position Highlights


HealthPlus Staffing is seeking a Board Certified or Board Eligible Family Medicine or Internal Medicine Physician for a full-time opportunity in Bakersfield, California.


The physician will provide comprehensive primary care to an older adult population within a PACE (Program of All-Inclusive Care for the Elderly) model. This role is designed for a physician who enjoys managing complex medical needs while working closely with an interdisciplinary healthcare team.


The incoming physician will play an important role in assessing participants, managing acute and chronic conditions, developing individualized treatment plans, and coordinating care across multiple disciplines.


Practice Details

  • Physician serves as a source of medical and geriatric expertise for the care team




  • Supervisory collaboration with Nurse Practitioners and/or Physician Assistants as applicable




  • Participation in quality-improvement initiatives and clinical care planning




Clinical Opportunity


The physician will provide comprehensive primary care and medical management for PACE participants, including:




  • Conducting initial and ongoing comprehensive medical assessments




  • Obtaining medical histories and performing physical examinations




  • Diagnosing and managing acute and chronic medical conditions




  • Ordering and interpreting laboratory and diagnostic testing




  • Developing and updating individualized treatment and care plans




  • Managing medications and adjusting treatment when clinically appropriate




  • Providing preventive care, health screenings, and health education




  • Evaluating changes in patients' physical, functional, and cognitive status




  • Coordinating specialty consultations and additional medical services when needed




  • Collaborating with the interdisciplinary care team to address medical, functional, and psychosocial needs




  • Educating patients and families regarding diagnoses, treatment plans, medications, and disease management




  • Coordinating care when assigned patients require hospitalization or skilled nursing care




  • Maintaining accurate and timely clinical documentation




  • Participating in quality assessment and performance-improvement activities

Qualifications




  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)




  • Board Certified or Board Eligible in Internal Medicine or Family Medicine




  • Must have an active California medical license




  • Must have an active DEA license




  • Must not be restricted from seeing Medi-Cal or Medicare patients




  • Minimum of one year of documented experience working with a frail or elderly population




  • Geriatric medicine experience or additional qualifications in Geriatrics preferred




  • Current CPR/BLS certification




  • Strong clinical assessment and chronic disease management skills




  • Ability to effectively manage medically complex older adults




  • Strong communication and interpersonal skills




  • Ability to work collaboratively within an interdisciplinary healthcare team




  • Commitment to compassionate, patient-centered care




Ideal Candidate


The ideal candidate is a Family Medicine or Internal Medicine physician who enjoys caring for older adults and is comfortable managing patients with multiple chronic conditions and complex healthcare needs.


The physician should value a collaborative approach to medicine and be comfortable working closely with an interdisciplinary team to coordinate comprehensive care for each patient.
