Primary Care Provider (NP/PA/MD/DO) – Mobile Primary Care

Base Salary: $150,000 + Bonus Eligibility

Billet Health is seeking an experienced and entrepreneurial Primary Care Provider to join our rapidly growing Mobile Primary Care division serving the Phoenix metropolitan area.

This is a full-time, Monday–Friday (8:00 AM–5:00 PM) opportunity for a provider who is passionate about delivering high-quality, patient-centered care directly in community and residential settings.

Practice Settings

Providers will deliver care in:

• Assisted Living Facilities


• Skilled Nursing Facilities


• Memory Care Communities


• Group Homes


• Private Residences


• Retirement Communities

Responsibilities

• Conduct comprehensive patient evaluations and physical exams


• Diagnose and manage acute and chronic illnesses


• Develop individualized treatment plans


• Prescribe medications and manage ongoing therapies


• Review diagnostics and coordinate follow-up care


• Perform minor procedures and wound care (preferred)


• Maintain timely and accurate documentation


• Collaborate with interdisciplinary healthcare teams

Qualifications

• Active Arizona license as an NP, PA, MD, or DO


• DEA license with prescribing authority


• Board certification where applicable


• Minimum of 1–2 years of clinical experience


• Family Medicine experience preferred


• Wound care experience strongly preferred


• Strong clinical judgment and ability to practice independently

Compensation & Benefits

• Base Salary: $150,000 annually
