Job Description - Psychiatric Care Coordinator - Registered Nurse

 


The Psychiatric Care Coordinator / Nurse (RN) plays a key role in supporting behavioral health services through clinical care coordination, patient follow-up, and interdisciplinary collaboration. This position focuses on ensuring continuity of psychiatric care, supporting patients across levels of treatment, and reducing gaps in care through proactive outreach and coordination. The Psych RN will also assist with clinical workflows, patient education, and administrative support to enhance the efficiency and quality of psychiatric services. In addition, this role will be responsible for medical case management of PCMH target populations whose needs often bridge psychiatric and physical medicine. The schedule for this role will be 8 AM-4 PM, Monday through Friday.


Responsibilities:  



  • Monitor and manage active psychiatric caseloads, including outreach to patients who have missed appointments or have not been seen within 90 days 

  • Conduct warm handoffs for patients transitioning to higher or lower levels of care (e.g., Intensive Outpatient Programs (IOP)

  • Perform RN-driven care planning visits for patient caseload in compliance with Patient-Centered Medical Home (PCMH) RN care planning requirements 



  • Coordinate care with external providers, including IOP programs, visiting nurses, and specialty services 



  • Obtain updates from IOP providers and support patients in re-establishing care following program completion 



  • Follow up on referrals to specialty services (e.g., neuropsychology, sleep medicine) to ensure continuity of care 



  • Facilitate release of information (ROI) requests and obtain relevant medical records from outside providers 



  • Serve as a primary point of contact for urgent patient calls and clinical concerns, triaging appropriately 



  • Conduct psychiatric intake work-ups for both in person and telehealth visits, including gathering clinical history and supporting documentation 



  • Assist providers in managing patient care needs, including lab coordination, medication monitoring, and follow-up care 



  • Support utilization of lab services, including urine toxicology (UTOX) and pharmacogenetic testing (e.g., GeneSight) 



  • Provide patient education on mental health conditions, treatment plans, and medication adherence 



  • Promote patient engagement and self-management through education and supportive interventions

Requirements:  



  • High School Diploma  



  • Unencumbered RN License in the State of Connecticut  



  • At least 1 year of clinical experience   



  • Experience maintaining a trauma-informed space  



  • Ability to relate to a broad range of people and communicate clearly, using tact, discretion, and sensitivity with patients  



  • References required for short-listed candidates  



  • A criminal background check absent offenses impacting ability to perform role

  • Active BLS certification 


  


Preferred requirements:  



  • Spanish language fluency  



  • At least 1 year of psychiatric nursing experience 



  • Bachelor of Science in Nursing 



  • At least 1 year of experience providing gender-affirming care  



  • Working knowledge of Epic (EHR)  



  • At least 1 year of experience providing HIV prevention and treatment services 


Compensation & Benefits:
