PT Home Care

University Home Care Inc is seeking a dedicated and compassionate Physical Therapist (PT) to join our home care team in the Southeast Michigan Region. Must be willing to travel.  As a PT Home Care provider, you will deliver high-quality, personalized rehabilitation services to patients in the comfort of their own homes, helping them regain mobility, improve function, and enhance their quality of life.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized treatment plans.


• Provide therapeutic interventions and monitor patient progress.


• Educate patients and their families on treatment plans and home exercises.


• Document all patient interactions and maintain accurate medical records.


• Collaborate with interdisciplinary care teams to coordinate patient care.


• Ensure compliance with all regulatory and company policies.

Requirements

• Valid Licensed Physical Therapist credential in the state of Michigan.


• Experience in home health care or outpatient physical therapy preferred.


• Strong communication and interpersonal skills.


• Ability to work independently and manage time effectively.


• Reliable transportation to travel to patient homes within the Southeast Michigan Region.
