Position Summary:
The Psychiatric Care Manager is responsible for coordinating and supporting the delivery of psychiatric services within Finger Lakes Community Health. This position manages psychiatric referrals, facilitates communication between patients, providers, and behavioral health professionals, and ensures timely follow-up and continuity of care. Working collaboratively with Psychiatric prescribers, psychologists, and the integrated care team, the Psychiatric Care Manager develops, implements, and monitors individualized treatment plans. The role also assists with psychiatric emergencies and serves as the primary contact for staff and patient concerns related to psychiatric services.
Main Responsibilities:
Referral Management
Manage and oversee all psychiatric referrals from initial intake through treatment, coordinating, monitoring, and directing providers to ensure timely access and seamless communication across the care team.
Serve as the primary point of contact for new psychiatric referrals, supporting both medical and psychiatric providers during the intake and care management process.
Care Coordination
Coordinate care and communication among Psychiatric prescribers, primary care providers, social workers, therapists, patients, families, and community resources.
Facilitate referrals to appropriate levels of care, including inpatient, outpatient, and community-based programs.
Manage transitions of care between settings to ensure continuity and patient safety.
Participate in multidisciplinary team meetings, case reviews, and care planning sessions to ensure all members remain informed of patient status and progress.
Operational Support & Crisis Management
Provide daily coordination and support to medical and psychiatric providers regarding referrals and ongoing patient care.
Serve as a central point of contact for staff and patient concerns related to psychiatric services at Finger Lakes Community Health.
Coordinate activities and communication with Finger Lakes Addiction Counseling and Referral Agency (FLACRA) co-located staff.
Support the call center and front-end staff with managing psychiatric emergencies, providing guidance on appropriate responses and escalation as needed.
Facilitate immediate intervention for patients in crisis, including coordinating emergency care or hospitalization.
Patient Advocacy & Education
Advocate for patients’ needs and preferences in all aspects of their psychiatric care.
Provide education and resources to patients and their families on psychiatric conditions, medications, treatment options, and coping strategies.
Monitor patient progress toward treatment goals using evidence-based assessment tools.
Identify potential risks or complications and take appropriate action to address them.
Adjust care plans and interventions as needed to optimize patient outcomes.
Quality Improvement
Participate in quality improvement initiatives to enhance psychiatric care coordination and outcomes.
Monitor performance indicators and contribute to the development of best practices in psychiatric care management.
Medical Record Maintenance
Document all patient interactions, referrals, care coordination, and follow-ups accurately within the electronic health record system, in accordance with Finger Lakes Community Health policies and procedures.
Chart and lock notes within timely manner in accordance with Finger Lakes Community Health policies and procedures.
Address telephone encounters, actions, and referrals in the electronic health record system in accordance with Finger Lakes Community Health policies and procedures.
Staff Supervision
Coordinate schedules for psychiatry team to ensure coverage and support efficient patient flow.
Supervise the performance of psychiatry team including conducting evaluations, auditing staff data entry, providing counseling, training, and administering disciplinary actions as needed.
Ensure staff are trained and performing in alignment with departmental standards and required competencies.
Other Duties
Perform other related duties as assigned by the supervisor.
Education
Bachelor’s degree in Nursing (BSN), Social Work, Psychology, or a related field required.
Master’s degree in Nursing (BSN), Social Work, Psychology, or a related field preferred.
Relevant experience will be considered in place of formal education.
Certification or diploma from an accredited nursing program required, certification in psychiatric nursing or as a Certified Case Manager preferred.
Qualifications
3+ years in psychiatric or behavioral health care acting as a subject matter expert and operating independently required.
Prior experience in a supervisory position preferred.
A valid government-issued photo identification document is required.
Current CPR certification through a HRSA accepted agency is required (can be provided upon employment).
A current, unrestricted, NYS Registered Nurse license is required.
Completion of continuing education in accordance to NYS licensing renewal and privileging requirements.
Strong knowledge of psychiatric disorders and treatment modalities.
Leads conversations and knowledge sharing across departments.
Has advanced expertise in facilitating team communication and conflict resolution.
Applies advanced reasoning in complex situations to bring solutions to team.
Demonstrates ability to initiate value-adding improvements.
Physical Requirements
Sitting/Standing: Prolonged sitting with occasional standing or walking.
Computer Use: Frequent typing and prolonged screen time.
Moderate Lifting: Regularly lifting or carrying items weighing between 15-40 pounds.
Visual: Clear vision for reading and inspections.
Hearing Clarity: Ability to hear conversations in person or in virtual communications.
Noise Sensitivity: Work may require filtering background noise to focus on speech or signals.
Work Environment
Health Center/Clinic: Work within a healthcare facility, interacting with patients and staff.
Remote: Home or remote office, using virtual communication to collaborate and interact with patients and staff.
Fast-Paced: Busy environment with multiple priorities to manage and changing business needs that require problem-solving skills.
Office Hours: Standard business hours, with occasional extended shifts based on clinic needs.
