Title: Quality Assurance Specialist

SUMMARY OF POSITION:
This position is responsible for quality assurance, accuracy and overall integrity of the care management records and documentation completed by the Health Home Department. This role is to ensure compliance with federal and state regulations and best practices through development of auditing tools. Analyzes collected audit data, identify trend for staff re-training and implementing corrective action plans in collaboration with Health Home management staff. Provides support to Directors, Managers and Supervisors to insure that all documentation and reporting requirements are prepared and maintained in a professional and well-coordinated manner.
ESSENTIAL FUNCTIONS
· Support the quality management program by performing QA tasks for Health Home Department pertaining to documentation of processes, including but not limited to progress note completion, case conferencing and care planning
· Ensure necessary documentation per Health Home program guidelines is made for clients in outreach
· Perform internal supplemental audits to verify that facility records files are in conformance to applicable Health Home program standards and regulations
· Conduct audits of inter-departmental workflows and provide any necessary recommendations to management based on findings
· Identify gaps in documentation and coordinate with management to arrange trainings as indicated.
· Perform protocol, data, and report inspections to verify Health Home program compliance with federal and state regulations and best practices
· Develop regular reports of findings and any recommendations to be communicated timely to management team
· Other duties as assigned by Quality Management Department

EDUCATION/EXPERIENCE:
· 5 years of experience working in QA in the healthcare field required, Health Home or related case management programs preferred
· Bachelor’s degree in related field required; Master’s in Public Health, Social Work, Mental Health Counseling preferred
· Computer literacy in Microsoft Office products and electronic health records, TREAT preferred
. Must be proficient in Excel (Pivot Tables, V/HLOOKUP functions)
