Quality Care Specialist/Physician Practices

Full-time, day shift, Monday-Friday, hours will vary
No Weekends or Holidays

JOB REQUIREMENTS

EDUCATION
Minimum: High school diploma or GED/HSE accredited by the State Board of Education.
Preferred:  

EXPERIENCE
Minimum:  One year experience in patient care.  Proficient in Microsoft applications and electronic medical records.
Preferred:  LPN experience with two years in healthcare with prior abstracting experience with knowledge of quality initiatives.

SUMMARY
-Assists with practice transformation efforts across primary care offices.
-Maintain knowledge and proactively identify new or changing quality metrics and initiatives so as to be a resource for information and interpretation for SMC.
-Responsible for accurate and timely reporting of quality data.
-Performs extensive medical reviews and chart preparation to ensure appropriate documentation, compliance with quality metrics, and care coordination for patients.
