Sage Group HealthCare Staffing is seeking RN Hospice for a 13-week travel / contract position in Grand Junction, CO. Accepting both local and traveler candidates.

Pay Rate: Range $2080 to $2120 Weekly Gross pay based on a 40-hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.50/mile for distance covered between patient home appointments

Unit: Home or Community 

Shifts available: Days

Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri with occasional back up for weekend and on-call. Contract offer flexible scheduling 

Position Requirements:

• Registered Nurse: 1 to 2 years - Required
  


• Hospice Nursing experience: 1 to 2 years – Required
  


• EMR/EHR charting - Required
  


• Must have valid Driver’s License and Personal Auto Insurance
  

Education, License and Certification Requirements:

• ASN or an equivalent – Required
  


• BSN or an equivalent – Highly preferred
  


• RN License for State applying or Compact if accepted in the state - Required
  


• BLS Certification from AHA or American Red Cross - Required
  

Job Description:

• Provide Nursing Care for Hospice Patients in private homes, nursing homes, group homes or assisted living facilities.
  


• Manage assigned caseload of patients assigned caseload of patients and lead the interdisciplinary team caring for those patients
  


• Provide assessments, care planning, pain management and symptom management and all other aspects of hospice nursing
  


• Administer Physician-prescribed medication
  


• Ensure timely and accurate submission of patient documentation and charting in adherence to client policies.
  


• Territories to cover: Grand Junction, CO and surrounding areas
