Regional Home Care Field Registered Nurse (RN) Supervisor (Temporary Contract)

FlexStaff is seeking a Home Care Field Registered Nurse Supervisor to work for one of our external clients, a non-profit organization that provides home healthcare services to the elderly, disabled and chronically ill in the New York metropolitan area. 

This is a temporary contract with possibility to become permanent.

Schedule: Monday - Friday 8:30am-5:30pm, (1 hour lunch)

In this role you will be responsible for conducting supervisory joint visits with Field Nurses in Brooklyn, Staten Island and Far Rockaway, NY, providing training/in-service education to professional clinical field staff and paraprofessional staff, performing nursing duties as assigned in the Diagnostic & Treatment Center (DTC) or Medical Practice. 

Requirements:

• Must have a NYS RN License. 

• Minimum of one (1+) year of experience in home care as a community health nurse in a Certified Home Health Agency, Long Term Home Health Care Program, and/or Licensed Agency. Minimum of 1-year experience working with the elderly.

• Supervisory experience in a community health settings, home care or MLTC

• Valid NYS driver’s license and reliable vehicle required

Responsibilities include, but not limited to: 

• Conduct audits and validate clinical documentation for accuracy, compliance, and alignment with evidence-based practice. 

• Perform supervisory field visits, assess competencies, and oversee paraprofessional staff. 

• Collaborate with care coordinators, partner organizations, and interdisciplinary teams to meet patient needs. 

• Ensure adherence to clinical protocols, regulatory standards, and infection control guidelines. 

• Supervise and guide CHNs, Personal Care Aides, and Home Health Aides. 

• Provide clinical leadership and operational oversight for assigned cases.

• Perform additional duties as assigned.
