Job Description - Regional Hospital Liaison

AristaCare Health Services
AristaCare is seeking a Regional Hospital Liaison to develop and strengthen relationships with hospitals and referral sources in Union County, NJ. This role is responsible for driving admissions, coordinating patient referrals, and promoting AristaCare’s skilled nursing and rehabilitation services.

Key Responsibilities:

Build and maintain relationships with hospital case managers, discharge planners, and physicians
Coordinate patient referrals and facilitate smooth admissions to AristaCare facilities
Conduct on-site hospital visits and represent AristaCare services
Track referral activity and support census growth initiatives
Collaborate with facility Admissions, Clinical, and Marketing teams

Qualifications:

Healthcare marketing, admissions, or liaison experience preferred (SNF or hospital experience a plus)
Strong communication and relationship-building skills
Knowledge of Union County hospital market preferred
Ability to travel locally within Union County
